Request Appointment
Start by clicking on the "BooK Now" button on the right, under the section that says: "Request Your TeleHealth Appointment Here". On the next screen, select an available appointment slot, complete the short form, then wait to receive your email confirmation.
Check Your Email
A link will be sent to your email address within 2 hours. Click on this link to access the Patient Portal where you can confirm your TeleHealth appointment and sign your Consent and HIPAA Agreements, as well as your initial patient health forms.
Request Appointment
At the appointed time, you will receive a Video call and your TeleHealth session begins.
