Request Appointment
Start by clicking on the Service that you want to be seen for. On the next screen, select an available appointment slot, complete the short form, then wait to receive your email confirmation.
Check Your Email
A link will be sent to your email address. Click on this link to access the Patient Portal where you can confirm your TeleHealth appointment and sign your Consent and HIPAA Agreements, and then complete your initial patient intake forms.
Request Appointment
At the appointed time, you will receive a Video call and your TeleHealth session begins.
